Cancellation policy and booking terms
Cancellation policy. Please give at least 24 hours' notice to cancel or reschedule an appointment. Late cancellations and no-shows are charged a $50 fee. Insurance does not cover these fees. If you arrive more than 15 minutes late, we will reschedule you.
- Booking a time slot is a request for an appointment and is subject to confirmation by our office. You may receive a confirmation via email or text message.
- You consent to receive appointment reminders and related communications by email, text message, or phone from Tampa Bay Dermatology and our scheduling partner.
- An appointment does not establish a physician-patient relationship until your visit takes place and you are seen by a provider.
- New patients will be asked to complete intake paperwork and provide insurance information prior to or at the time of their visit.
- Your personal information is handled in accordance with our Privacy Policy and HIPAA Notice of Privacy Practices.
